Failed to take Medicare deduction on terminated employee

As I was preparing my W-2’s I noticed that somehow the Medicare box was not checked so all the checks that were produced for an employee did not have Medicare taken out. The employee is no longer employed.  I need to correct my 941 for 3rd Qtr.  1. How do I do this and 2. Do I need to make sure the W-2 then shows his wages and payments for Medicare and how do I do this?   Never had this happen…
