System Admin making adjustments

We just recently had a major screw up with our payroll, it was off by $200K when I searched the audit, it showed that System Admin made adjustment to payroll checks from 2021. Has anyone else had this type of issue?

It shows up when QuickBooks Online makes automatic adjustments, typically when changes to connected transactions, bank account mappings, or accounting preferences trigger related updates to payroll checks.
