1. Go to the Gear icon, then select Custom Form Styles.
2. You can edit a template or create a new one by clicking the New Style and choosing the form you want.
3. Hover to the Content tab, then select the Header section.
4. Click the + Address, then uncheck the Street Address box.
   
5. Tick the Hide Country, then Done.
