Why do I get "Authentication Failed"?

This site, quickbooks.intuit.com, is rather buggy and Intuit steadfastly refuses to do anything meaningful about it.

I was getting the same thing earlier today and have from time to time in the past. I finally deleted all of the cookies that included "intuit" and "quickbooks", restarted my browser, logged in again, and then it worked as expected.
