I am paying £20 VAT for quick books. Do you have to pay extra for pay role?

It seems you are using QBO Essentials. Payroll is an add-on module and you have to pay it. If your budget is limited, consider downgrading your plan to QBO Simple Start or switching to another accounting app + a 3rd party payroll app with a lower fee.
